This editorial from Darin S. Evans raises important questions regarding the decision to stop mass drug administration (MDA) for countries with onchocerciasis as they approach transmission interruption. New guidelines for stop MDA have to be operationalized, taking into account the challenges of analysis of historic baseline data and treatment decisions, assessing onchocerciasis transmission in post-treatment settings for lymphatic filariasis MDA, and capturing sufficient numbers of the black fly vector to make a stopping decision.
